Terms & Conditions

The terms and conditions of Building Blocks Therapy are set out to confirm that both parties understand the services to be carried out, the costs involved, the time scales set for payments and data protection.


All client contact details and sensitive data information provided by or related to a client is always treated as strictly confidential. Building Blocks Therapy will never intentionally divulge, disclose or communicate client information without written or verbal consent by those with parental responsibility or a child with capacity.

Assessment Packages
By completing the consent form, personal information sheet, check list and contract returning it; you are entering into a contractual agreement for assessment, review or treatment sessions provided by Building Blocks Therapy. By scheduling assessment packages, you are agreeing for them to take place within the home. Clinic sessions may be available for additional cost which can be provided upon request. Your child is expected to attend the session with an adult with parental responsibility.


Treatment Sessions

By scheduling treatments sessions, you are agreeing for them to take place within the home. Clinic sessions may be available for additional cost which can be provided upon request. Your child is expected to attend the session with an adult with parental responsibility.


Occupational Therapy treatment can be agreed on completion of the initial assessment and completion of the report. Treatment sessions will be goal driven which will be agreed with the child and adult with parental responsibility. Goals will be reviewed at the end of each treatment period, before proceeding with agreement of the parents / carers/child. Treatment session reports and programme writing are chargeable activities.


Tribunal:

Tribunal services are provided at the discretion of the OT and will depend on timescales and availability of the therapist. Where appropriate a quote for this service will be provided based upon individual needs and prices of this service may vary.


Fees:

You will be invoiced upon booking for 50% of the assessment, review or treatment session fee; this payment should be received 72 hours prior to assessment, review or treatment session.


You will be invoiced for the outstanding balance with the completion of the written report or when the assessment has been completed in full. Full payment should be received within 14 working days of the invoice date.


You will be invoiced upon booking for a tribunal service for  50% of the agreed fee; this payment should be received 72 hours prior to assessment, review or treatment session. Building Blocks Therapy does not currently accept Health Insurance Policies as payment.


Terms of settlement are stated on invoices and are 14 working days. If payment is not made within this period and following reminder emails, we will seek to recover charges by other means.


We welcome payment by bank transfer, cheques will be considered upon request. Please note that any collection costs incurred by Building Blocks Therapy (including fees billed by collection agencies, credit reference agencies and solicitors) will be added to the final amount payable.


Mileage over 15 miles will be charged at an additional rate of 45p per mile.


Safety:

An adult with parental responsibility is expected to be present during assessment, review or treatment sessions. The therapist has no responsibility to look after additional siblings present in assessment, review or treatment sessions.

Adults with parental responsibility are expected to be responsible for their children when not in the direct contact of the OT (outside of any treatment area provided) whether in the home or clinic setting. In the family home, the area provided for the Therapist to work within should be free of sharp objects on the floor, clean and any pets removed from the vicinity, and placed in another room. Please do not smoke in the presence of the child or the therapist.


Adults with parental responsibility are responsible for children / family members attending the Clinic setting and their safety inside and outside the building.


The Therapist will treat the child / adult at any venue with respect and use appropriate language and dress code. If an interpreter is required for assessment, review or treatment sessions it is the responsibility of the client to arrange this. It is expected that the adult will address the Therapist with equal respect and wear appropriate clothing and use appropriate language. Any physical, verbal or emotional abuse towards the Therapist by the adult at any time will result in the termination of the session. Children whose condition involves potential for harm (either physical, verbal or emotional) either towards themselves or others should be notified to the Therapist in advance. The child should wear loose comfortable clothing such as T-shirt and shorts or a tracksuit. Clothing such as: tight jeans, hoodies and tops with additions such as sequins, beads or fringing are not suitable for therapy.


Equipment

The therapist will provide equipment for assessment, however when in the home environment may request to use appropriate toys to assist with sessions. Consumable items such as play dough, therapy brushes, oral motor toys, which will need to be purchased by the parent / carer, on the advice of the OT, if to be used on a regular basis within therapy sessions. This does not include the assessment package. This is due to this type of item being only suitable for single child use, where hygiene and infection control regulations recommend that it is not re-used with other children. OT may recommend that specific equipment as outlined in the Assessment Report should be purchased by parents/carers or schools.


Insurance

The Therapist is insured with Royal College of Occupational Therapists.


Disclosure and Barring Scheme

The Therapist has a valid Enhanced Disclosure DBS document which is viewable upon request.


Training / Registration

The Therapist is registered with the Health Professions Council, as a Practicing member of the Allied Health Professions – Occupational Therapist. This requires proof of ongoing professional development as a practitioner, and re-registering every 2 years. This is the only registration which allows practice as an Occupational Therapist in the UK, and requires the Therapist to hold valid qualifications for registration.


The Therapist has valid documentation for up to date skills and knowledge for: Basic life support and Anaphylaxis, Safeguarding Children and Adults, Manual Handling, infection control, information governance, health safety and the Mental Capacity Act. The OT abides by the Code of Ethics as detailed by the Royal College of Occupational Therapists and will follow local policies within schools or colleges, where necessary.


Cancellation of Appointments

If you cannot make a session please let us know. We operate the following cancellation policy:


Cancellation 48 hours prior to assessment, review, or treatment session will incur no admin charge*, and you will be offered another appointment or your deposit returned.


Cancellation with less than 48 hours to assessment, review, or treatment session will incur an admin charge of £15 and your deposit will not be returned.


A no show to an appointment will incur an admin charge of £15 and you will be liable for the full price of the service. This will also include fuel costs if we have not previously been advised your child is unavailable and the therapist has travelled to a venue.


We understand that there are exceptional circumstances therefore if we are able to do so, we may, at our discretion, waive cancellation fees, but this is not guaranteed.


If you have to cancel, please do so in person by phone – an email must be deemed not received unless we acknowledge it. Voicemail messages left will be responded to ASAP.


If you need to contact your therapist urgently in-between sessions please email building-blockstherapy@outook.com or telephone 07743734510 and leave a message. Therapist will return your call as soon as possible.


Record Keeping:

Any documentation will be stored safely in accordance to Caldicott Guidelines (rcot.co.uk) and be treated as confidential. Alternatively, documentation may be kept by the parent / carer and brought to each session for progress notes to be added.


A copy of our GDPR Privacy Notice is issued to all new clients and can be seen upon request.


Progress notes will be kept following any intervention or contact made with the child / family / school.


*Admin fee: We set aside large blocks of reserved time for your appointment, especially for assessment, review and treatment planning, note writing and programme development which incurs an admin charge.
